South cashes the S6, squeezing West in three suits.  Unable to throw a Heart or Club which immediately lose those suits, West pitches the D6, pitching the HQ from the Dummy.

Unfortunately, now declarer will now run the DQ with the D10 falling.  Declarer can now finesse the DJ to declarer's D9, subsequently winning the DK.
